Levels of Care

What are the levels of care that are provided at Ecumen CountrySide?

Ecumen CountrySide offers Independent and Assisted Living apartments. We provide a variety of services including assistance with personal care, medication administration, homemaking, and laundry.

Who determines the proper transition plan should a resident need to move to another level in the Continuum of Care?

After the initial assessment, there is an ongoing 90-day review process on all of our residents who receive services. If a change in condition warrants a need for additional services, the resident, family or guardian, and caseworker collaborate to determine the services needed. A full assessment is performed each time there is a significant change of condition or hospitalization to ensure that the resident receives the appropriate level of services. In the event that a transition to a higher level of care is needed, our nursing and leadership teams will work directly with families to assist in the transition.

What are the health care services in Assisted Living?

We provide a large menu of a la carte health services. Each resident will receive an individualized care plan, which may change as their needs change. Also, we have a partnership with The Smart Clinic to provide an on-site Nurse Practitioner who can see our residents and provide services at Ecumen CountrySide. Health insurance would cover these visits, the same as if going to an outside clinic.

If I decide to move to Ecumen CountrySide, how do I decide what health care services are needed?

Once a decision is made to move to Ecumen CountrySide, our nurse will meet with you and your family members to decide together what health care services are needed. When receiving health services at Ecumen CountrySide, we work closely with families, physicians, therapies, and other care professionals to provide the best continuity of care.

How does my primary care physician fit into my health care program at Ecumen CountrySide?

When receiving health services at Ecumen CountrySide, you may see any provider of your choice. Our team approach to care relies on open communication to bring all disciplines together to develop the best possible outcomes for the health and well-being of our residents.

How does “health care” work in Assisted Living?

Ecumen CountrySide provides a large menu of a la carte health services to choose from. With our flexible services, families are also welcome to be as active in the care of their loved one as they wish. The goal of our health services program is to be there when you need us.

What is the availability of the nursing staff?

Ecumen CountrySide’s Clinical Director is primarily available Monday-Friday during business hours. Our Licensed Practical Nurse is primarily available during business hours as well. There is always a Registered Nurse on call 24 hours a day, 7 days a week to answer all clinical questions from Resident Assistant staff.

Apartment Living and Admissions

What are the services included in the monthly rent?

Ecumen CountrySide offers many services included in the rent; weekly housekeeping, afternoon snack and coffee hour, eight round trip transportation rides within city limits, all utilities, Wi-Fi, 24 hour resident assistance and all recreational programs.  Meal plans and cable are available for an additional fee.

What are the options for senior apartments at Ecumen CountrySide?

Ecumen CountrySide provides Independent and Assisted Living apartments featuring full kitchens with dishwashers. Some units have washers and dryers. Overall, Ecumen CountrySide offers eight floor plans to choose from, ranging from one-bedroom units, one-bedroom plus a den and two-bedroom units.

If I have a loved one who wishes to move to Ecumen CountrySide, where should I begin?

Do you allow pets? Can I bring my dog or cat with me?

Yes. Small pets are welcome in our community, however a resident must be comfortable providing all of the care needs for their pet. There is a $800 non-refundable pet fee for all pets that move into our building.

Can I rent a garage?

Yes. There are garages on site. Please call our Marketing Manager at 507-461-9277 for availability.

Is there a waiting list at Ecumen CountrySide?

Yes. Ecumen CountrySide does have a waiting list. Please talk with our Marketing Manager at 507-461-9277 for more information on how the waiting list works.

Activities

What is there to do once I make the move to Ecumen CountrySide? Will I remain active?

You may remain as active as you wish. We offer a menu of activities to meet the social, spiritual, physical, educational, and creative needs of residents. A copy of our Activity Calendar is available on our website for you to review.

What types of activities are available to help keep my loved one engaged?

Our full Activity Calendar offers many events each month. Also, an extensive program of activities lead by our Life Enrichment Coordinator meets the social, spiritual, physical, educational, and creative needs of residents. Are there religious activities I can choose to participate in?

Ecumen CountrySide offers a devotion/service located in the Community Room two-three days per week. Ecumen CountrySide also has a Life Enrichment Coordinator to coordinate spiritual care programs and church services. We also offer a variety of spiritual programs including personal visits with a pastor, Catholic Mass, Lutheran service, Ecumenical hymn sings and one-on-one encouragement.

Does Ecumen CountrySide offer any type of exercise or wellness classes?

Ecumen CounrtySide partners with a nationally known fitness program called Live 2B Healthy. A trained fitness instructor leads classes three times per week that are geared to improve balance, strength, and flexibility of older adults. Our Activity Staff also provide a variety of physical fitness options. Please see Activity Calendar for more details.

Memory Care

How is it determined when a resident needs to move from Assisted Living to Memory Care?

Our Assisted Living nursing team will recommend when Memory Care is appropriate. When the resident needs a specialized, safe and secure care environment due to the progression of Alzheimer’s or other forms of dementia, we will assess them and then meet with the resident and family to discuss the options that memory care can provide.

Does Ecumen CountrySide offer Memory Care?

At Ecumen CountrySide, we do our very best to provide assistance to our residents in every stage of life. For those individuals who require a secured environment for their safety, we recommend our sister community Ecumen Brooks, which is located on the north side of Owatonna, just four miles away. Ecumen Brooks offers 12 apartments for those who need extra services in a secured neighborhood within the building.

Dining

Do you offer accommodations for special diets?

Ecumen CountrySide works hard to meet the special dietary needs of residents and can accommodate most requests.

How many meals are included in the monthly rent?

At Ecumen CountrySide, meals are separate from the monthly rent. All three meals are prepared and served daily, and can be purchased for an additional fee.

Are there meal options?

There is an alternative menu that residents may order from if they wish to eat something different than what is being served. A monthly menu is provided to all residents.

How often are the menus changed and distributed?

The menus are changed quarterly with the seasons. They are distributed to all residents on the first of the month.

Can a resident bring a guest to a meal?

Guests are always welcome to join us for a meal. However, there is an additional cost for guests to attend a meal. If a guest is to join us for a meal, dining services must be notified two hours before the meal time.
